| Medical Authorization Form |
| | This form is required by anyone under the age of 18 to play in Jacksonville Ultimate League.
The form enables parents or guardians to authorize the provision of emergency treatment for their children who are injured or become ill while under the authority of a chaperone in the event that they cannot be reached.

| UPA Juniors Chaperone: Consent and Release form |
| | This form is required for each team that has a player under the age of 18. Is recommended that each team have one Chaperone per 12 youth members.

| Accident Report Form |
| | This form is for all organizers and captains. You should have a copy of this form with you at all league events.
If someone was to get injured this form would need to be filled out and mailed to the UPA.
